PERSOVENTA: Personalized Preventive Prescription

At the FHLIP 2025 conference, I presented “Empowering Health Through Personalized Preventive Prescription”—a snapshot of my PERSOVENTA project, an initiative designed to help shift preventive healthcare in Canada from reactive treatment to proactive, personalized prevention.

The project integrates validated assessment tools with a dynamic knowledge base linking risk factors to diseases, aiming to enhance EMRs with prevention-relevant data for earlier action by patients and clinicians. Covering 15 health domains, it translates preventive insights into practice using AI pathways to bridge research and clinical workflows while addressing health inequities. Since launching Phase 1 in June 2024, over 65 clinicians and researchers have collaborated to refine evidence-based tools, with a focus on rigorous validation of reliability and guideline adherence. This foundational work supports a phased approach from tool identification to detailed questionnaire analysis, ultimately creating a knowledge-driven infrastructure for AI-supported predictive analytics in preventive care at scale.

MAKING IT PERSONAL: AI, KNOWLEDGE, AND THE FUTURE OF HEALTHCARE

I recently joined Blake on the For What It’s Worth podcast to discuss the future of AI in healthcare and the importance of making technology truly personal for clinicians and patients (Personalized Medicine). In our conversation, we explored how knowledge management, digital innovation, and personalized medicine can transform care delivery, while also addressing the challenges of trust, adoption, and equity. Showcasing Innovations at the HIMSS Eastern Canada Chapter Conference

I am honoured to have presented our research on the Preventive Assessment Tool (PAT) at the HIMSS Eastern Canada Chapter Conference. This event offered an excellent platform to connect with leaders in digital health and explore potential collaborations to transform preventive healthcare.

The PAT project showcases the dedication of over 45 volunteers, including researchers, clinicians, and data scientists. We are focusing on two key components:

1. Validated Preventive Assessment Tool (PAT) Package: We developed Canadian-validated tools for 16 critical health topics, aligned with clinical guidelines to tackle Canada’s top 30 preventable diseases.

2. Evidence-Based Knowledge Base: A data-driven resource mapping risk factors to diseases, supporting personalized preventive care through AI insights.

This initiative aims to fill a crucial gap in healthcare by providing personalized preventive prescriptions. I am grateful to the HIMSS Eastern Canada Chapter for the opportunity to showcase PAT, and I am proud of our team’s commitment to advancing preventive healthcare. I look forward to fostering sustainable partnerships to improve health outcomes and personalized care.

Championing Personalized Medicine as the Future of Healthcare

In an interview with Dr. Jordan Vollrath, Chief Medical Officer of the Cherry Health Podcast, I shared my perspective on Personalized Medicine as the ideal model for healthcare. This approach emphasizes delivering the right clinical intervention—whether it is diagnosis, treatment, or prevention—at the right time for the right individual. During the conversation, I highlighted how I apply this philosophy in my full-time role at OntarioMD, where I lead initiatives that embed personalized medicine into digital health strategies. I also discussed how this approach shapes my teaching at the University of Toronto (UofT) and George Brown College (GBC) and my ongoing research projects to advance preventive healthcare. This holistic application of personalized medicine underscores my commitment to transforming healthcare into a more precise, proactive, and patient-centred system.

Shaping the Future of Personalized Preventive Healthcare

In a recent interview with Saul Marquez, CEO of the Outcomes Rocket podcast, I had the opportunity to share my vision for personalized medicine and preventive healthcare. The conversation highlighted my current research project, in which I lead efforts to develop an innovative data-collection tool and a comprehensive knowledge base. This initiative aims to support the creation of AI-driven algorithms that enable Personalized Preventive Prescriptions tailored to individual patient needs. By advancing this work, I seek to contribute to a future where preventive care is more precise, proactive, and patient-centred.

Driving Innovation in Digital Health and AI

In an interview with Ana Maria Palusci, Chief Creative Officer of the Health Tech Horizons Podcast, I had the chance to share my vision for digital health and AI from multiple perspectives. The discussion highlighted my contributions across various roles, including applying the Personalized Medicine approach within OntarioMD initiatives, my teaching efforts at the University of Toronto (UofT) and George Brown College (GBC), and my leadership in personal research projects focused on personalized medicine. I also spoke about my role as Executive Director of the Future of Health Leadership, Informatics, and Policy (FHLIP) Conference, which we are preparing to launch for the first time. This experience reflects my ongoing commitment to advancing digital health, education, and research to shape the future of healthcare innovation.

My vision: Fully AI-integrated primary care workflow

I was privileged to participate in the HEAL Conference 2024 (Healthcare Enhanced by AI & Learning) at Western University alongside my esteemed colleague Dr. Keith Thompson. Our presentation focused on the current landscape of AI-enabled solutions in the healthcare market and how these tools can support both physicians and patients. Representing OntarioMD, I also shared my vision of a fully AI-integrated primary care workflow. I firmly believe that “AI will never replace physicians — but physicians who use AI will replace those who don’t.” This conference was an excellent platform to discuss the transformative potential of AI in healthcare and its role in enhancing clinical practice.


Fascinating Day at the T-CAIREM 2024 Symposium!

I had an enriching experience at the T-CAIREM Symposium 2024 held at the University of Toronto, Hart House. The focus on Multimodal AI in healthcare was truly enlightening. Discussions centred on how integrating various data types (medical imaging, electronic records, biosensors, etc.) can revolutionize patient care and diagnostics. Multimodal AI holds the potential to pave the way for transformative advancements, potentially leading to Artificial General Intelligence (AGI) – machines with human-level understanding – and eventually Artificial Super Intelligence (ASI), which could revolutionize healthcare beyond our imagination.

The potential of AI-powered doctors to analyze vast amounts of patient data for personalized diagnoses and treatment plans was particularly fascinating. These advancements in personalized medicine could drastically improve patient outcomes and healthcare delivery.

Overall, the symposium left me inspired by the immense potential of AI to transform healthcare delivery and excited for the future of health with Multimodal AI!
